PATIENT PHONE CALLS

URGENT CALLS are attended to immediately by the doctor. Non urgent calls are usually returned on the same day, but always within 24 hours.

PRESCRIPTIONS AND REFERRALS

We encourage patients to attend for prescriptions and referrals. On the unusual occasion when this is not practical a fee of $24.00 applies.

RECALL REMINDER ENROLMENT

Patients of this practice are automatically enrolled into a Reminder Recall System. Recall letters are sent when a preventative medical action is due e.g. papsmear, cholesterol etc. or when a follow-up examination is required e.g. diabetes check, a general health examination etc. Patients may elect to opt out of this system after discussing this with their doctor.

PRACTICE FACILITIES

Parking is available on Glenferrie Rd, next to The Hawthorn Library and also at the rear of our clinic. There is also a vehicle drop off next to our clinic entrance with a disabled access ramp.

NON-SMOKING PRACTICE

In the interest of public health no smoking is permitted on the premises as well as in the garden or carpark.

TRANSFER OF FILES TO OTHER PRACTITIONERS

A copy of patients’ medical records will be transferred on receipt of a request in writing and payment of $35.00 (which covers cost of copying and handling).

CONFIDENTIALITY OF RECORDS

Your medical record is a confidential document. It is the policy of this Practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff. We abide by the 10 National Privacy Principles available at http://www.privacy.gov.au/health/index.html.
